TEACHERS/ADVISORS: If you are bringing more than 1 team to the MN State Envirothon, please complete this form for each team that is attending.

LUNCHES: The Envirothon Coordinators regret that we cannot accommodate all the potential dietary needs and wishes of every event participant. Box lunches will be provided and served with beverages. However, if any team member or advisor has special dietary restrictions, they should plan on bringing their OWN lunch to the event. Please remind your students to be respectful, and clean up after themselves also.

ALTERNATE STUDENTS: Alternate Students listed MUST have competed at the Area competition. Alternates are listed as a means to substitute an original team member, if an emergency arises. We encourage you to list up to 2 alternates if available. However, we do request that alternates only attend if absolutely necessary.
